Every three years those who hold active (and now retired) credentials with Atlantic Coast Conference will need to submit two forms in order to receive updated Ministerial Wallet Cards and a credential extention of three years.
Forms must be submitted for credentials to be renewed. Please return these forms to the ACC office by Friday, September 19, 2025. Please don’t hesitate to reach out to Conference Coordinator Brook Musselman or your conference minister for additional assistance with any piece of this process. Background Certifications
Visit the pa.gov Get a Clearance webpage to begin the process of completing the Pennsylvnia Child Abuse History Clearance, the Pennsylvania State Police Criminial History Clearance, and the FBI Criminal History Clearance.
These certifications are valid for five years, so to ensure that they are valid for this credentialing cycle (through Oct 2028), all certifications should have been completed since October 2023. Each minister must file a new covenant form with dates of completed certifications. Updated copies of these three certifications should be maintained by your home congregation unless other arrangements have been made with the ACC office.
Instructions for Completing Certifications (Clearances)
1. Visit https://www.identogo.com/
2. Click the button, Get Fingerprinted in the upper right corner.
3. Select state > Go.
Under Enrollment Services, select Digital Fingerprinting.
2. In the search bar, enter service code 1KG756.
5. Schedule an Appointment then follow the form prompts to schedule a fingerprinting session at a location near you.
1. Submit and pay for the certification through the EPATCH website.
2. Choose, “Submit a New Record Check”.
3. Follow the prompts and begin an “Individual Request”.
4. Follow the prompts to complete the form and checkout.
1. Submit and pay for the certification as an employee through the CWIS self-service portal.
2. Login as an individual or create an individual account.
3. Follow prompts to login, complete the form and checkout.

This form must be completed by all actively credentialed leaders and signed by a congregational representative who has reviewed the credentialed leaders’ original background check results.
